Case Management services are provided to adults 18 years or older with a diagnosed mental health disorder and/or co-existing deficits in one or more areas of functioning. Clients in need of Case Management Services generally have a history with the mental health system. These clients often have multiple losses in their immediate support system and numerous needs that require a variety of services and/or resources to meet those needs.
Case Management Services are designed to help the individual achieve an optimal level of independence by coordinating training in the areas of basic living skills, conflict resolution, problem solving, medication management, socialization and finding and maintaining employment if appropriate. These services are provided in the individuals natural environment and include assessments and referrals that link clients with a variety of community resources. Clients are monitored regularly to ensure an ongoing continuity of care is maintained, newly identified needs are addressed and progress is being achieved toward treatment plan goals.
Case Management Services are community based and provided throughout Collier County. Adults can access these services in Naples and Immokalee, but must meet certain clinical criteria to enter into the program determined by a clinician through a comprehensive clinical assessment.
